Mixed a separate problem it found into the requested work

Situation: Partway through a requested task, the AI finds a different problem.
What the AI did: It listed its fix for that problem among the options as if it were part of the requested task. The person couldn't tell whether the fix was needed for the request, and work had to be redone.
How to ask next time (the operator's example): "Write what the requested task needs and the separate problems you noticed in different sections. For each separate problem, say in the first line whether it can wait."
One question to ask: Is this needed for the request, or is it a separate problem?
Check it yourself: Go through the AI's report and mark each item "needed for this request" or "found along the way." Decide whether the request is done from the first group only.
